Cornerstone Oral Surgery Center accepts patients by referral from a dentist or physician. If you’re interested in our services, use the form below to let us know — we’ll follow up and coordinate with your referring dentist to get your referral started. We’ll respond by phone or email within one business day.
For Referring Dental Practices
To submit a patient referral, call us directly at 605-413-6510 or use the contact form and note that you are a referring practice. We’ll confirm receipt, contact your patient, and provide updates throughout the care process.
